Packets are provided in English and in Spanish. They include information on Informed Consent, Power of Attorney for Health Care, Living Will (end-of-life decisions), Advance Health Care Directive, Comfort One - DNR/CPR Directive, and Organ Donation.

Home-Delivered Meal Programs

The Community Based Home Care Programs provide service to adults 19 or older who are at risk of institutional placement. It includes case management, personal care, and homemaking. This is a cost shared program funded by the State of Wyoming and fees are determined on a sliding scale according to income.

Long-Term Acute Care (LTACH)

A long-term acute care hospital is a specialty-care hospital designed for patients with serious medical problems that require intense, special treatment for an extended period of time – usually 20-30 days. An LTACH offers more individualized and resource-intensive care than a skilled nursing facility, nursing home or acute rehabilitation facility.
