Requests for copies of medical records can be submitted in person or electronically. You must present photo identification before we will release a copy of your records. If the patient is a minor (younger than 18), a parent or the legal guardian must sign the authorization and present photo identification. There is no charge to patients who request a copy of their medical records. Please allow up to 30 days for all requests.

The Health Information Management Department at Memorial Hospital of Sweetwater County maintains and retrieves patient medical records. Patient Privacy is a priority at MHSC. We take pride in protecting your medical records.
Urgent patient care requests may be faxed to 307-352-8175 between 6 a.m. and 5 p.m. Monday through Thursday and between 6 a.m. and 4:30 p.m. on Friday. (For after-hours and weekend requests, please call the main hospital number at 307-362-3711 and have the house supervisor paged.)
Copies of birth certificates must be requested through Wyoming Vital Records. Their telephone number is 307-777-7591.
