Planning for Healthcare Decisions: Advance Directive packets are provided in
English and in
Spanish. They include information on Informed Consent, Power of Attorney for Health
Care, Living Will (end-of-life decisions), Advance Health Care Directive,
Comfort One - DNR/CPR Directive, and Organ Donation.

The Community-Based Home Care Programs provide service to adults 19 or
older who are at risk of institutional placement. It includes case management,
personal care, and homemaking. This is a cost shared program funded by
the State of Wyoming and fees are determined on a sliding scale according
to income.

Long-Term Acute Care (LTACH)
A long-term acute care hospital is a specialty-care hospital designed for
patients with serious medical problems that require intense, special treatment
for an extended period of time – usually 20-30 days. An LTACH offers
more individualized and resource-intensive care than a skilled nursing
facility, nursing home or acute rehabilitation facility.

Skilled Nursing Facility (SNF) are for patients who need to get better
from an illness or surgery. Patients with long-term needs for ongoing
nursing care can also receive care in an SNF.
