Planning for Healthcare Decisions: Advance Directive packets are provided in English and in Spanish. They include information on Informed Consent, Power of Attorney for Health Care, Living Will (end-of-life decisions), Advance Health Care Directive, Comfort One - DNR/CPR Directive, and Organ Donation.

      • The church has a small pantry with help offered by appointment. Call 307-362-3530, leave your name and how many you are feeding at home. Messages are checked Monday-Thursday afternoons.
      • Food can be dropped off during office hours from 2:45-4:30 p.m. Mondays and 12:30-4:30 p.m. Tuesday, Wednesday & Thursday.
    • Food Bank of Sweetwater County
      • Rock Springs Food Bank: Open Second & Fourth Thursdays of the month from 10 a.m. to 2:45 p.m., 90 Center Street. 307-382-7332.
      • Green River Food Pantry: Open First & Third Thursdays of the month from 10 a.m. to 4:45 p.m., 550 Uinta Drive. 307-875-9600.
    • Hand Up Food Cart

      • Open noon to 6 p.m. weekdays. Closed weekends and on the Second and Fourth Thursday of the month so individuals can use the Food Bank of Sweetwater County.

      • A full meal is provided from noon to 1 p.m. weekdays for anyone who needs it. All food and work is donated.

    Home-Delivered Meal Programs

    The Community-Based Home Care Programs provide service to adults 19 or older who are at risk of institutional placement. It includes case management, personal care, and homemaking. This is a cost shared program funded by the State of Wyoming and fees are determined on a sliding scale according to income.

    Long-Term Acute Care (LTACH)

    A long-term acute care hospital is a specialty-care hospital designed for patients with serious medical problems that require intense, special treatment for an extended period of time – usually 20-30 days. An LTACH offers more individualized and resource-intensive care than a skilled nursing facility, nursing home or acute rehabilitation facility.
