Welcome to HealtheLife

Your health information with the hospital and its Specialty Clinics is available through one portal – HealtheLife. It offers a single electronic medical record to better serve our patients' needs.

Creating Your Account

There are two ways to create your patient portal account:

1. When you check in for an appointment at the hospital or one of its clinics, the registration staff will ask for your email address in order to send you a link to sign up for the portal. Simply follow the instructions in that email.

Follow the instructions. Complete the appropriate information.
Once the account is created, you will be taken to the main page of your patient portal where you can access your patient information.

Some tips

• When asked how you would like to verify your account, you have the option of “Personal Identifier” or “Email Address.” Using “Personal Identifier” and entering your SSN is the best way to verify.
• When setting up your username, you cannot use your email address.
• As you begin to create your password, tips will pop up below the box to let you know how many and what type of characters are required.

What Information is Available?

• Health record information including medications, allergies and immunizations
• Test results including labs and radiology
• Visit summaries
• Health histories
• Helpful website links such ac the Centers for Disease Control and Prevention, WebMD, and the hospital's Facebook page.

What Else Can I do?

• Review health information and report any incorrect information to your healthcare team
• Send a message to my healthcare team
• Request a medication refill
• Request an appointment with a clinic provider
• See past and upcoming appointments
• Download health information
• As parents, you can set up the portal for minor children and link them to one account
• Caregivers also may be given access to a patient's portal to assist in the patient's healthcare
• Pay your bill

There's An App

On your device, go to your App Store and search for HealtheLife. Find Memorial Hospital of Sweetwater County, and let the app prompt you from there. You can now take HealtheLife on the go.
